Keeping an accurate list of medications can save your life

It is very important for everyone to keep an updated and accurate listing of all medications.  This universal medication form should be shared with all your health care providers (family care physician, pharmacist, cardiologist, OB/GYN, physical therapist, dentist, etc).  All of your health care providers, especially your pharmacist, should be made aware of all the medications you are currently taking.  You should include all prescription medications, over the counter medications, vitamins and herbal supplements.

Your pharmacist is your best ally in keeping an updated universal medication form.  By taking new medicine prescriptions and refills to the same pharmacy, your pharmacist can make sure that all of the medicines are working together.  Using multiple pharmacies raises the risk that you may be doubling up on some medications or drug interactions.  If you do use more than one pharmacy, make sure that you share an updated universal medication form with you at all times.

The key to maintaining medicine safety is to download this free universal medication form, fill it out, keep it updated, keep it with you at all times and share it with all of your health care providers.
