• Letter of intent to include the following:
• Discuss why you wish to complete a PGY2 informatics specialty residency program.
• Discuss why you are interested in completing the PGY2 informatics specialty residency at U of L Health Care
• Share your perceived strengths and weaknesses.
• Share your short-term goals for the residency year.
• Discuss your long-term pharmacy career goals.
• Discuss aspects of prior training that demonstrate preparation for the program.

• Curriculum vitae:
• Include descriptions of pertinent employment history, Pharm.D. training and experiences, PGY1 residency training and experiences, formal presentations, curricular activities, computer skills, publications, honors or awards, and activities demonstrating leadership potential.

• Letters of recommendation:
• Three completed and signed recommendation forms are to be submitted directly. Please use the American Society of Health-System Pharmacists’ Residency Applicant Recommendation Request Form.

• Official transcripts from accredited College of Pharmacy
