Your Patient Identification Band
Each patient gets an identification (or "ID") band when checking into University Hospital. The band contains important information about you that helps us meet your individual needs. If you are not wearing your band, your meals and other services may be delayed. Please wear the band throughout your stay. If your band is removed, falls off, or becomes uncomfortable, let your nurse know immediately.
Patient Meals
If your physician restricts you to a modified diet, the registered dietitians of Patient Food Services will help you understand your specific diet and will provide information about restrictions, menu planning and cooking techniques.
Food brought to you by visitors -- regardless of the type of diet you are prescribed, you should have your nurse check any food brought to you by your visitors to make sure it's allowable as part of your dietary plan.

Advanced Directives
If you are 18 or older and mentally competent, you have the right to make decisions about your medical care. Advanced Directives are legal forms that help you do that.
There are three kinds of Advance Directives:
- A Living Will lets you state your wishes about medical care, or choose another person(s) to make medical decisions for you if you lose the ability to do so.
- A Health Care Power of Attorney lets you name a person to make health care decisions for you if you become unable to decide for yourself.
- An Advance Instruction for Mental Health Treatment allows you to give instructions and preferences about mental health treatment. You may select someone to make these decisions for you if you lose the ability to decide for yourself. This document automatically expires in two years.
If you already have a Kentucky directive, we must have a copy in order to follow your wishes. If you cannot give us a copy, we will be happy to complete another form for you, in case one is needed while you are in University Hospital.
If you have an Advance Directive from another state, it cannot be used in Kentucky. We will be happy to help you complete a Kentucky form so that your wishes can be followed.
If you want to complete an Advance Directive, or have questions, talk to your nurse, physician, patient and visitor relations representatives, or staff in Chaplaincy Services. Your nurse can also help you find someone to notarize your form.
Environmental Services (Housekeeping)
Your Room is cleaned daily by our Environmental Services Associates. If your room is not cleaned to your satisfaction and there are other special cleaning needs, please do not hesitate to contact our Guest Services Patient Hot Line at extension 3013.
Billing Information
Whether your insurance will cover all costs - including those billed separately from your doctor depends on many factors. Business office personnel will gladly assist in filing your claim, but, ultimately, you are responsible for your account. For help in filing insurance claims, call the business office at 502-562-3225, Monday through Friday, 10:00 a.m. - 4:00 p.m. or visit our on-line Billing Center.
If you are enrolled with Medicare, please note that Medicare may not always be the primary carrier responsible for payment of your bill. Other insurance coverage (group, hospice, etc.) may need to be billed before Medicare. Please notify the Admitting Department at 502-562-3011 should you need to change or correct any billing information.
University Hospital provides financial counselors who are available to assist you if you need to make arrangements for payment. You may contact the financial counselors by dialing 502-562-3226 8:00 a.m. - 4:30 p.m. Monday through Friday.
