- Discuss why you wish to complete a PGY1 pharmacy residency
- Discuss why you are interested in completing the PGY1 pharmacy residency program at U of L Health Care
- Share your perceived strengths and weaknesses
- Share your short-term goals for the residency year
- Discuss your long-term pharmacy career goals
- Curriculum vitae:
- Include descriptions of pertinent employment history, clinical learning experiences, formal presentations, curricular activities, computer skills, honors or awards, and activities demonstrating leadership potential.
- Letters of recommendation:
- Three completed recommendations are to be confidentially sealed in a signed envelope and included in the resident application packet. Please use the American Society of Health System Pharmacists’ Residency Applicant Recommendation Request Form.
- Official transcript from accredited College of Pharmacy:
- If not included at application time, most recent semester must be presented at on-site interview.
Application packet must be received by January 9, 2009.

Only complete application packets received by the above deadline will be reviewed for selection of on-site interview. Selected candidates will be contacted for an on-site interview to take place in late January and early February.
